Bethany has a form for you to fill out with caregiver and emergency contact information. When you send it in, we will be able to know how to reach you in case of emergency. If you know of a church member or regular attendee who is not an Internet user, please let Office Manager Maria Mar know and we will send a paper copy by mail for him or her to fill out and return.
